If an employee enrolls within 30 days of eligibility, a physical is not required, and the carrier cannot refuse enrollment.
An employee can waive life insurance coverage and apply at a later time. However, the employee must complete an evidence of insurability form with the late application. The carrier may request an employee to submit to a physical exam in order to be considered for late enrollment. The carrier also has the right to reject enrollment.
