Financial Aid Authorization

The federal government requires the Financial Aid Office to obtain authorization from students before using federal aid (e.g., Pell Grant, SEOG Grant, TEACH Grant, Perkins Loan, and/or federal student loans) to pay for certain charges to your account other than tuition, comprehensive and laboratory fees, room, and meals. Examples of charges that we must obtain authorization for include but are not limited to books, residence hall damage fees, late fees and residence hall key fees.  Similar authorization must be obtained from parents who borrow federal PLUS loans.

A Financial Aid Authorization Etrieve form has been created for students, while an authorization form parents borrowing Parent PLUS loan will be emailed to the borrower or it can be downloaded.  For both students and parents, there are two sections to the form:  the top section related to current year charges and the bottom section pertains to prior year charges.  For each section, please indicate whether you are

  • Authorizing the use of federal aid for certain other charges for the duration of the student’s enrollment
  • Authorizing the use of federal aid for certain other charges until a specific date (provided by student or parent borrower)
  • Decline the authorization
