Documentation Guidelines

Academic Accommodation Documentation

Students with learning disabilities should provide a copy of your grade 12 IEP or 504 Plan and the most recent copies of educational and psychological reports. Testing must be from chronological age 16.5 or later. The student must present a diagnosis or a disability as defined by the Americans with Disabilities Act (ADA) and Section 504 of the Rehabilitation Act of 1973.

Students requesting academic accommodations who are also under the care of a physician, clinician or other specialist should have their treating clinician complete and submit the Monmouth University Academic Provider Form below.

Meal Plan Accommodation Documentation

Students requesting meal plan accommodations who are also under the care of a physician, clinician or other specialist should have their treating clinician complete and submit the Monmouth University Meal Plan Form below.

Housing Accommodations Documentation

Students requesting an accommodation in Monmouth University Housing must complete the following steps:

  1. Complete the Residential Life Housing Contract.
  2. Complete the Confidential Registration form.
    • If you are currently registered with Disability Services, submit a Supplemental Request in your Accommodate file.
  1. Complete the student part of the DDS Request for Student Housing Contract Sections A & B.
  2. Provide appropriate documentation by submitting a completed Disability Provider Information Form- Housing Documentation. This form must be completed by a licensed professional.


ESA (Emotional Support Animals) Accommodation Documentation

  1. Complete the Confidential Registration form

**If you are currently registered with Disability Services, submit a Supplemental Request in your Accommodate file. 

  1. Read the Service and Emotional Support Animals Policy. Once you understand the section about emotional support animals, complete and return the Signature Form for ESA Request Process & Procedure.
  2. Complete the student part of the DDS Request for Student Housing Accommodations Sections A&B.
  3. Provide appropriate documentation by submitting a completed Disability Provider Information Form- ESA Documentation. This form must be completed by a licensed professional.

In Section 6 on page 2 of the form, your clinician should note that you are requesting an ESA as a housing accommodation based on a disability related need/diagnosis, and the type of animal you are requesting.  Since there is limited room in this section, they also need to submit a detailed rationale on their letterhead that includes information about the established relationship you have with the animal, length of time you’ve owned the animal, its therapeutic value in the treatment of your diagnosis, and any other information that supports your request for this accommodation.

  1. When all documentation has been received meet with DDS to discuss the request.

Documentation must be in PDF format. Documentation not in PDF format will cause delays in processing.
