Description
The Registered Physical Therapist ensures Physical Therapy services are delivered appropriately, meeting regulations. Provides quality Physical Therapy services in a timely manner to patients according to a written plan of care.
Professional Duties
- Maintain member confidentiality at all times (HIPAA).
- Provides direct planned patient therapy services (within scope of practice) under the direction and orders of a Physician/Practitioner and according to agency policies and patient plan of care.
- Completes initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate times.
- Develops and revises the plan of care, in consultation with the physician/practitioner and other team members as appropriate, which establishes goals and incorporates therapeutic, preventative and rehabilitative therapy services as appropriate.
- Provides an evaluation of DME assignments/needs as appropriate.
- Advises and consults with patient, family, and other team members as appropriate regarding safety, plan and outcome of care, and discharge planning.
- Provides health teaching to patient and/or caregiver regarding treatment and plan of care. Education will utilize terms and teaching methods that are customized to the learning needs of the patient and/or caregiver.
- Provides accurate and timely documentation consistent with the plan of care, this may include preparing clinical and progress notes as appropriate. Documentation will be completed in a timely manner and according to the organization’s documentation expectations.
- Performs supervisory visits of Therapy Assistants and Home Health Aides as appropriate.
- Participates in team meetings and in-service programs as required.
- Weekend shift coverage as needed.
- Performs other duties as assigned.

Qualifications
- High School Diploma or equivalent. Bachelor’s (BSPT), Master’s (MPT), or Doctor of Physical Therapy (DPT) degree in Physical Therapy required.
- Current California Physical Therapy License.
- Minimum of 1 to 2 years recent acute care experience required. 1-2 years in Home Health preferred.
- Basic computer skills.
- Ability to manage time and stay organized.
- Good communication skills, detail oriented, critical thinking and problem solving skills. Excellent customer service and telephone etiquette skills.
- Current California Driving License and proof of insurance.
- BLS required within 60 days of hire.
