Description

The Dementia Clinic Assistant will collaborate with the Dementia Clinical Pharmacist to support the Dementia Clinic. This role will function as a non-licensed specialist, providing expertise in resources, care coordination, and escalation to the clinician for members with Alzheimer’s dementia.

 

Professional Duties

  • Contact patients and patients’ families to follow up on the Clinical Pharmacist’s recommendations.
  • Document patient and family interactions in the electronic medical record under the minimum supervision of the Clinical Pharmacist.
  • Oversees lower acuity level patient needs by performing needs assessment and applying program interventions.
  • Monitors and interprets patients’ and families’ needs by identifying families in crisis and follows protocols to ensure that their needs are met.
  • Communicates professionally and compassionately via telephone and via email.
  • Makes referral to appropriate community-based services and applies non-pharmacologic management of dementia-related behaviors and caregiver education/support.
  • Meets weekly with the Clinical Pharmacist to summarize interventions instituted for these patients.
  • Works collaboratively with the Clinical Pharmacist to ensure that patient acuity assignments are accurate and up-to-date.
  • Perform other duties as assigned.

 

Qualifications

  • Associate’s degree required. Bachelor’s degree preferred.
  • 1 year of experience in a healthcare setting required. 2 years of experience with dementia-related programs strongly preferred.
  • Working knowledge of human biological systems.
  • Strong interpersonal and communication skills, with the ability to engage passionately with patients and families.
  • At the discretion of DOHC/FHC management, this position has the potential to be a full or hybrid telecommuting position.

Hearing/Visual Acuity Adequate for use with computers, telephone and/or cellphone. Approximately 50% of day.

Computer Highly technical work environment. Must be able to work minimum of 6 hours a day using keyboard, mouse and monitor.

Reaching Above head 75 degrees approximately 55% of day.

Hand grip dexterity Approximately 55% of day.
