Are you passionate about helping patients regain independence and improve their quality of life? Santana Hills Healthcare Center is seeking a compassionate and motivated Occupational Therapist (OT) to join our rehabilitation team.

Santana Hills Healthcare Center is a 167-bed skilled nursing and sub-acute facility in San Jose, CA. We are committed to delivering high-quality, patient-centered care and creating a supportive environment where healthcare professionals can grow and thrive.
What You’ll Do:
- Evaluate patients’ physical, cognitive, and emotional abilities
- Develop individualized treatment plans and therapy goals
- Provide therapeutic interventions to improve daily living skills and independence
- Educate patients and families on therapy techniques and adaptive equipment
- Maintain accurate documentation and progress notes
- Collaborate with physicians, nurses, caregivers, and interdisciplinary team members
- Monitor patient progress and adjust treatment plans as needed
Required Qualifications:
- Bachelor’s or Master’s Degree in Occupational Therapy
- Current Occupational Therapist License in California
- Strong communication and interpersonal skills
Preferred Qualifications:
- Experience in rehabilitation, geriatric, skilled nursing, or sub-acute settings
- CPR Certification
