Intent: <\/strong>To outline the process of requesting a Late Test Room Booking.<\/p>\n\n\n\n

Accountability: <\/strong>The Director of DDS and all DDS professional staff, the ADA\/504 Coordinator and students with disabilities requesting DDS accommodations and support services.<\/p>\n\n\n\n

Applicability:<\/strong> Students registered with the Department of Disability Services (DDS) approved for Extended Test Time.<\/p>\n\n\n\n

Procedure: <\/strong>Students requesting a Late Test Room Booking must complete the following steps:<\/p>\n\n\n\n

  1. Register with the Department of Disability Services (DDS) and be approved for Extended Test Time.<\/li>\n\n\n\n
  2. Email professor and copy Department of Disability Services on that email: examsdds@monmouth.edu<\/a> including:\n
      \n
    1. Request testing accommodation\/s.<\/strong><\/li>\n\n\n\n
    2. Include course code.<\/li>\n\n\n\n
    3. Provide date and start time of exam.<\/li>\n\n\n\n
    4. Indicate how much time the class is given to complete the test.<\/li>\n\n\n\n
    5. Include any special instructions (notes, books, computer).<\/li>\n\n\n\n
    6. Indicate how the professor will provide the exam (email, drop off).<\/li>\n\n\n\n
    7. Provide how the professor wishes to receive the completed exam returned (scan\/email, pick up, student delivery).<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n\n\n\n
