Our mission is to enhance the well-being of our communities,\u00a0individuals\u00a0and families through responsive,\u00a0innovative\u00a0and collaborative human service\u00a0<\/p>\n<p><strong>About the Role:<\/strong>\u00a0<\/p>\n<p>The Community Clinician provides assessment, treatment planning, and therapeutic services to adult clients in their homes or community settings, with a primary focus on older adults experiencing transportation or mobility barriers. This role helps ensure individuals have access to high-quality mental health and substance use services where they are most comfortable. Working closely with the Council on Aging and an interdisciplinary clinical team, the Community Clinician enjoys the flexibility and independence of community-based work while\u00a0benefiting\u00a0from a collaborative team environment, supportive supervision, and dedicated office space for documentation and care coordination.\u00a0<\/p>\n<p><strong>Responsibilities:<\/strong>\u00a0<\/p>\n<ul>\n<li>Provide evidence-based, individualized psychotherapy in accordance with best practices.\u00a0<\/li>\n<li>Conduct intake assessments, evaluations, diagnostic assessments, treatment planning, and referrals as clinically\u00a0indicated.\u00a0<\/li>\n<li>Adhere to program philosophy and established admission, continued stay, and discharge criteria.\u00a0<\/li>\n<li>Complete clinical documentation, recordkeeping, and data collection\u00a0in accordance with\u00a0agency policy and the requirements of state, federal, payer, and\u00a0accrediting\u00a0bodies.\u00a0<\/li>\n<li>Complete documentation\u00a0in a timely manner, including treatment plans, progress notes, timesheets, managed care authorizations, discharge summaries, and other required records.\u00a0<\/li>\n<li>Coordinate follow-up care, aftercare planning, referrals, and discharge services to support continuity of care.\u00a0<\/li>\n<li>Facilitate client access to community resources and\u00a0additional\u00a0supports.\u00a0<\/li>\n<li>Provide targeted, short-term case management services as clinically\u00a0appropriate.\u00a0<\/li>\n<li>Deliver services in the client&#8217;s preferred setting, primarily through home visits and community-based locations.\u00a0<\/li>\n<li>Collaborate effectively with internal departments and external community partners, including the Southwestern Vermont Council on Aging, to coordinate client care.\u00a0<\/li>\n<li>Participate in regular clinical supervision, consultation, team meetings, department meetings, and agency meetings; seek guidance and support as needed.\u00a0<\/li>\n<li>Maintain professional standards of conduct, ethics, confidentiality, and compliance with agency policies and procedures.\u00a0<\/li>\n<li>Provide education and training to staff in areas of expertise as requested.\u00a0<\/li>\n<li>Participate in community outreach and educational activities as requested by the supervisor.\u00a0<\/li>\n<li>Perform other duties as assigned.\u00a0<\/li>\n<\/ul>\n<p>\u00a0<\/p>\n<p><strong>Qualifications:\u00a0<\/strong>\u00a0<\/p>\n<ul>\n<li>Masters Degree in Social Work, Clinical Mental Health Counseling, Counseling, Psychology, or a closely related field required.\u00a0<\/li>\n<li>Licensed\u00a0preferred;\u00a0License Eligible (Rostered as a Non-Licensed Psychotherapist) in the State of Vermont\u00a0required.\u00a0<\/li>\n<li>Knowledge of the assessment and treatment of mental health, substance use, and co-occurring disorders.\u00a0<\/li>\n<li>Knowledge of and sensitivity to the unique behavioral health needs of older adults and geriatric populations.\u00a0<\/li>\n<li>Ability to provide recovery-oriented, person-centered counseling and support to clients and their families in community-based settings.\u00a0<\/li>\n<li>Ability to\u00a0establish\u00a0therapeutic relationships with clients from diverse backgrounds while maintaining\u00a0appropriate professional\u00a0boundaries.\u00a0<\/li>\n<li>Strong clinical assessment, treatment planning, and crisis intervention skills.\u00a0<\/li>\n<li>Excellent written and verbal communication skills, including\u00a0timely\u00a0and\u00a0accurate\u00a0clinical documentation.\u00a0<\/li>\n<li>Strong organizational and time management skills with the ability to prioritize competing responsibilities and manage an independent caseload.\u00a0<\/li>\n<li>Ability to work both independently and collaboratively as part of an interdisciplinary team.\u00a0<\/li>\n<li>Ability to provide high-quality, client-centered services in a fast-paced environment with limited resources.\u00a0<\/li>\n<li>Proficiency\u00a0with electronic health records and standard computer applications.\u00a0<\/li>\n<li>Must have a valid driver\u2019s license, and reliable\u00a0transportation with personal automobile liability\u00a0insurance coverage at or above the levels mandated by the State of Vermont.\u00a0<\/li>\n<li>*Successful completion of a criminal history background check after\u00a0hire.\u00a0<\/li>\n<\/ul>\n<p><strong>Benefits:\u00a0<\/strong>\u00a0<\/p>\n<ul>\n<li>Competitive\u00a0Pay Range &#8211;
