{"id":45293,"date":"2026-07-06T19:53:32","date_gmt":"2026-07-06T19:53:32","guid":{"rendered":"https:\/\/www.monmouth.edu\/career-development\/job\/community-services-program-assistant\/"},"modified":"2026-07-25T08:27:28","modified_gmt":"2026-07-25T12:27:28","slug":"community-services-program-assistant","status":"publish","type":"ch_job","link":"https:\/\/www.monmouth.edu\/career-development\/job\/community-services-program-assistant\/","title":{"rendered":"Community Services Program Assistant"},"content":{"rendered":"<p><strong>Portage County Health &amp; Human Services <\/strong>has a full-time<strong> Community Services Program Assistant <\/strong>opening!<\/p>\n<p><i>Pay range starts at $19.95 \/ hr<\/i><\/p>\n<p><strong>What are the primary job duties?<\/strong><\/p>\n<p><i>Provision of independent living skills, household organization skills, money management, and assistance with medication reminders for consumers with Serious and Persistent Mental Illness enrolled in the Comprehensive Community Services Program.<\/i><br \/>\u00a0<\/p>\n<ul>\n<li>Teach specific skills such as money management, community accessibility,\u00a0laundry, meal planning, \u00a0employment related skills, maintaining personal hygiene, and cleaning.<\/li>\n<li>Organize and monitor day-to-day schedule when necessary.<\/li>\n<li>Provide direction for organizing and scheduling household duties and routines.<\/li>\n<li>Provide resources to consumers and their families for obtaining necessary household equipment.<\/li>\n<li>Assist consumers and their families in providing medication reminders to take medication as prescribed.<\/li>\n<li>Provide transportation to consumers for attendance of medical appointments, appointments related to their service plan, or for activities designated on the consumer\u2019s service plan.<\/li>\n<\/ul>\n<p><i>Engage in the coordination of care for consumers on assigned caseload.<\/i><br \/>\u00a0<\/p>\n<ul>\n<li>Regular contact with contracted caregiving service to provide updates on consumers and assure smooth transition to evening, weekend, holiday coverage for consumers and their families.<\/li>\n<li>Weekly to daily contact with client\u2019s assigned CCS service facilitator to obtain clinical updates regarding clientele.<\/li>\n<li>Attend weekly staffing for clinical consultation with CSP\/CCS\/Mobile Crisis\/Medical Director<\/li>\n<\/ul>\n<p><strong>What are the minimum qualifications?<\/strong><br \/>\u00a0<\/p>\n<ul>\n<li>High School Diploma or equivalent required.
