Additionally, this position will provide case management for high-risk pregnancies. This position assists women to enter prenatal care in early pregnancy, provides or refers for case management, assists in determining Medicaid eligibility, medical, nutritional, educational, social, and emotional needs.<\/p>\n<p>\u00a0<\/p>\n<p>\u201cPublic Health is a first responder agency for natural disasters (e.g. hurricanes, tornadoes, floods, winter storms), naturally occurring infectious disease outbreaks (e.g. influenza, SARS), technological hazards (hazardous materials releases, critical infrastructure disruptions), and terrorist incidents.\u00a0 This position, like all other positions within the Department, may be required to participate in any emergency response activities as deemed necessary by the Health Director or his\/her designee.\u00a0 Availability during emergencies and exercises is required.\u201d<\/p>\n<p>\u00a0<\/p>\n<p><strong>Examples of services performed<\/strong><\/p>\n<ul>\n<li>The PHN II will provide outreach to the community by means of presentations, displays and handouts on the importance of early and continuous prenatal care as well as health department services available.\u00a0<\/li>\n<li>This position will attend and participate in the agency staff conferences, workshops, and in-services as deemed advantageous by the PHN and her or his supervisor. The PHN II will function on agency committees and participate in program audits and evaluations.<\/li>\n<li>The PHN II will function in the maternal health clinic working directly with clients providing history taking, assessing client needs, and making appropriate referrals to available community resources. Providing maternal health education, HIV counseling, smoking cessation, and other educational needs as deemed appropriate for the client. Appropriate follow-up is ensured for abnormal labs and missed appointments.<\/li>\n<li>The PHN II will provide pregnancy test counseling and screen clients for presumptive eligibility for Medicaid and refer to community resources as applicable.<\/li>\n<li>Screening clients for presumptive eligibility Medicaid and refer to other community resources as applicable.<\/li>\n<li>Participate in agency staff conferences, workshops and in-services as deemed advantageous by the SWII and her supervisor.\u00a0 This position will function on agency committees and participate in program audits and evaluations.<\/li>\n<li>Must work with considerable independence in the clinical or home setting.\u00a0 Work will be reviewed in conferences with the Women\u2019s Health Lead Nurse and by case review.<\/li>\n<li>Complete timely documentation of visits, contacts, normally within 72 hours of contact.\u00a0 Adhere to agency policies and procedures<\/li>\n<li>The PHN II will maintain records and reports of services suitable for billing and data collection, adhering to program guidelines. The PHN II will have a working knowledge of local and state program policies and procedures. The PHN II will be expected to work with considerable independence in the clinical setting, with work being reviewed by the nursing director.<\/li>\n<\/ul>\n<p>\u00a0<\/p>\n<p><strong>Recruitment Standards:<\/strong><\/p>\n<p>Knowledge, Skills and Abilities \u2013 Knowledge and skill of nursing care with expertise in Women\u2019s Health; supervision of clients; administrative skills to develop the program; organize, implement and evaluate the program; teaching skills; knowledge of community resources and organizations and the ability to coordinate these as needed relating to public health.
