We believe that what happens early in life truly matters, and we are passionate about providing high-quality, innovative early childhood education and therapeutic services to every child in our community.<\/p>\n<p>\u00a0<\/p>\n<p><strong>SCOPE:\u00a0<\/strong>As a member of the Birth to Three, Early Support for Infants and Toddlers team, the Family Resources Coordinator (FRC) will provide coordination of services to families of children with developmental\u00a0delays,act\u00a0as\u00a0a\u00a0source\u00a0for\u00a0community\u00a0resourcesand\u00a0maintain\u00a0necessarypaperwork\u00a0and timelines and record keeping, supporting the family and therapist. The FRC will advocate for the child and\u00a0family\u00a0needs,and\u00a0serve\u00a0as\u00a0the\u00a0primarycontact\u00a0to\u00a0families,\u00a0facilitating\u00a0relationship\u00a0to\u00a0the\u00a0familyand therapists in guiding the Individual Family Support Plan (IFSP) and ensuring all services are provided in regulation to Part C services.<\/p>\n<p>\u00a0<\/p>\n<p>ESSENTIAL\u00a0DUTIES\u00a0AND\u00a0RESPONSIBILITIES<\/p>\n<p>Program\u00a0Planning\/Record\u00a0Keeping<\/p>\n<ul>\n<li>Take\u00a0the\u00a0lead\u00a0for\u00a0each\u00a0assigned\u00a0child\u00a0and\u00a0family\u00a0ensuring\u00a0appropriate\u00a0program\u00a0implementation<\/li>\n<li>Maintain\u00a0the\u00a0statedata\u00a0management\u00a0systemand\u00a0agency\u00a0medicalrecord\u00a0system\u00a0to\u00a0access\u00a0funding<\/li>\n<li>Participate\u00a0in\u00a0community\u00a0meetings\u00a0related\u00a0to\u00a0networking\u00a0for\u00a0program\u00a0visibility\u00a0information\u00a0on community resources and promotion of Early Intervention services<\/li>\n<li>Interact\u00a0with\u00a0other\u00a0Encompass\u00a0programsand\u00a0staff\u00a0to\u00a0support\u00a0family\u00a0knowledge\u00a0and\u00a0participation\u00a0in other organization programs<\/li>\n<li>Document\u00a0all\u00a0parentcontacts,\u00a0complete\u00a0any\u00a0necessary\u00a0reportsand\u00a0records\u00a0to\u00a0maintain\u00a0accuratefiles for program evaluation and monitoring for their caseload<\/li>\n<li>Facilitate\u00a0the\u00a0Individualized\u00a0Family\u00a0Service\u00a0Plan\u00a0(IFSP)\u00a0for\u00a0each\u00a0family\u00a0that\u00a0will\u00a0providing\u00a0supportto child\/family and therapist.<\/li>\n<li>Work\u00a0collaboratively\u00a0with\u00a0the\u00a0therapists\u00a0and\u00a0family\u00a0to\u00a0ensure\u00a0that\u00a0services\u00a0are\u00a0provided\u00a0in\u00a0natural environments, including inclusive playgroups, the household and community settings<\/li>\n<li>Ensure\u00a0compliance\u00a0of\u00a0Encompass,\u00a0IDEA\u00a0Part\u00a0C,\u00a0and\u00a0schooldistrict\u00a0policies,\u00a0procedures,\u00a0and\u00a0timelines<\/li>\n<li>Other\u00a0duties\u00a0as\u00a0assigned<\/li>\n<\/ul>\n<p>Coordinating\u00a0Services<\/p>\n<ul>\n<li>Work\u00a0with\u00a0the\u00a0ESIT\u00a0team\u00a0as\u00a0an\u00a0active\u00a0participant\u00a0to\u00a0coordinate\u00a0servicesand\u00a0offer\u00a0teamingsupport\u00a0to staff<\/li>\n<li>Work\u00a0alongside\u00a0FamilyResource\u00a0Coordinators\u00a0as\u00a0a\u00a0team\u00a0to\u00a0ensureresponsiveness\u00a0to\u00a0familiesand ESIT compliance<\/li>\n<li>Advocate\u00a0for\u00a0familiesto\u00a0assure\u00a0coordination\u00a0between\u00a0Encompass\u00a0servicesand\u00a0other\u00a0communityor organization services<\/li>\n<li>Maintain\u00a0knowledge\u00a0of\u00a0available\u00a0communityresources\u00a0and\u00a0make\u00a0necessary\u00a0referrals\u00a0to\u00a0meet<\/li>\n<\/ul>\n<p>families\u2019\u00a0needs<\/p>\n<ul>\n<li>Coordinate\u00a0with\u00a0medicaland\u00a0health\u00a0providersas\u00a0needed<\/li>\n<\/ul>\n<p>\u00a0<\/p>\n<p>Direct\u00a0Service<\/p>\n<ul>\n<li>Serve\u00a0as\u00a0firstcontact\u00a0for\u00a0incomingfamilies\u00a0to\u00a0the\u00a0program\u00a0&#8211;\u00a0complete\u00a0intakeprocess\u00a0for\u00a0the scheduled initial evaluation<\/li>\n<li>Schedule\u00a0interpreting\u00a0servicesas\u00a0requested<\/li>\n<li>Review\u00a0program\u00a0information\u00a0parent\u00a0rights\u00a0with\u00a0the\u00a0family,providing\u00a0them\u00a0with\u00a0written\u00a0information<\/li>\n<li>Refer\u00a0to\u00a0evaluation\u00a0team\u00a0for\u00a0evaluations\u00a0and\u00a0assessments<\/li>\n<li>Facilitate\u00a0the\u00a0development,\u00a0review\u00a0and\u00a0evaluation\u00a0of\u00a0the\u00a0Individualized\u00a0Family\u00a0Service\u00a0Plan\u00a0(IFSP). Track service delivery<\/li>\n<li>Facilitate\u00a0transition\u00a0plan\u00a0to\u00a0Part\u00a0B\u00a0or\u00a0private\u00a0servicesprior\u00a0to\u00a0child\u2019sthird\u00a0birthday\u00a0to\u00a0ensure\u00a0familyis connected to relevant services or resources when Early Intervention services end<\/li>\n<li>Offer\u00a0support\u00a0to\u00a0families\u00a0throughhome\u00a0visits\u00a0and\u00a0required\u00a0monthlycontact<\/li>\n<li>Demonstrate\u00a0a\u00a0desireto\u00a0identify,\u00a0gain\u00a0knowledge\u00a0in\u00a0and\u00a0awarenessof\u00a0individual\u00a0experiences\u00a0and societal oppressions that impact ESIT families and children in underserved populations centered around race, ethnicity, sexual orientation, belief systems, physical\/mental\/neurodevelopment differences, marital status, and gender<\/li>\n<\/ul>\n<p>\u00a0<\/p>\n<p>QUALIFICATIONS<\/p>\n<ul>\n<li>Bachelor\u2019s degree in Early Childhood, Human Services or related field; or at least two years of experience working or volunteering in the field of human services or early intervention, preferably with\u00a0home\u00a0visitation;\u00a0or\u00a0at\u00a0least\u00a0two\u00a0years\u00a0of\u00a0equivalent\u00a0life\u00a0experience\u00a0with\u00a0early\u00a0intervention\u00a0as\u00a0a parent\/caregiver.
