A Service Coordinator (SC) ensures the delivery of outlined client services for multiple programs, supervises direct reports, oversees operation of assigned households, and serves as the primary contact between the guardian, client, and HIL.<\/p>\n<p>This role works with clients directly and has office time to review care plans. Direct Care experience is strongly preferred.<\/p>\n<p>\u00a0<\/p>\n<p>As a Service Coordinator, you will:<\/p>\n<p>\u00b7 Develop and manage individual service plans (ISP) for assigned clients<\/p>\n<p>\u00b7 Ensure delivery of services outlined in ISP for client population across multiple programs<\/p>\n<p>\u00b7 Respond to program emergencies including those outside normal business hours<\/p>\n<p>\u00b7 Ensure medical treatment supplies are available within programs and direct care staff are trained on administration and logging of required treatments<\/p>\n<p>\u00b7 Serve as primary contact for hospital staff regarding emergency treatments and hospital stays<\/p>\n<p>\u00b7 Oversee household operations including: maintenance reports, menu development, grocery shopping, housekeeping, vehicle maintenance, and activity planning<\/p>\n<p>\u00b7 Perform HR and payroll functions for direct reports including: interviews, orientations, training, performance monitoring and evaluation, coaching, and corrective action<\/p>\n<p>\u00b7 Manage client funds and valuables in a responsible manner<\/p>\n<p>\u00a0<\/p>\n<p>Benefits:<\/p>\n<p>\u00b7 Salary up to \u00a0$50,000\/year<\/p>\n<p>\u00b7 Health, Dental, and Vision Insurance<\/p>\n<p>\u00b7 PTO after 60 days<\/p>\n<p>\u00b7 401k with ESOP benefits<\/p>\n<p>\u00b7 Referral Bonus Program &#8211; $2,000 Bonus per positive referral<\/p>\n<p>\u00b7 Tuition Reimbursement and Loan Pay-down Programs<\/p>\n<p>\u00b7 Employee Assistance Programs<\/p>\n<p>\u00a0<\/p>\n<p>Qualifications:<\/p>\n<p>\u00b7 High School Diploma or equivalent, Bachelor\u2019s preferred<\/p>\n<p>\u00b7 Valid Wisconsin driver\u2019s license with 3 years of acceptable driving record<\/p>\n<p>\u00b7 Personal vehicle with insurance<\/p>\n<p>\u00b7 3 years experience in long term care setting and 2 years supervisory experience<\/p>\n<p>\u00b7 Excellent communication and organizational skills<\/p>\n<p>\u00b7 Flexibility, autonomy, responsiveness, and a high level of initiative<\/p>\n<p>\u00b7 Ability to travel to different work locations, training sessions, and meetings<\/p>\n<p>\u00a0<\/p>\n<p>HIL is a MyPath Co. with locations across Wisconsin.
