As a key member of the hospice team, you will deliver quality end-of-life care in a supportive and empathetic manner, focusing on comfort, symptom management, and emotional support. You will work collaboratively with physicians, social workers, chaplains, and other healthcare professionals to ensure a dignified and peaceful transition for patients.<\/p>\n<p>\u00a0<\/p>\n<p><i><strong>Primary Responsibilities and Duties:<\/strong><\/i><\/p>\n<ul>\n<li>Conduct thorough assessments of patients; physical, emotional, and spiritual needs.<\/li>\n<li>Develop and implement individualized care plans in collaboration with the patient, family, and other hospice staff.<\/li>\n<li>Administer medications, manage pain and symptom control, and perform medical procedures as needed.<\/li>\n<li>Monitor and document patient progress, changes in condition, and response to treatments.<\/li>\n<li>Educate patients and their families on disease progression, care options, and symptom management.<\/li>\n<li>Collaborate with physicians, therapists, and other healthcare professionals to coordinate and optimize patient care.<\/li>\n<li>Offer emotional and psychological support to patients and families, addressing their concerns and facilitating discussions about end-of life care.<\/li>\n<li>Coordinate with other members of the hospice team to ensure comprehensive and continuous care.<\/li>\n<li>Advocate for patients\u2019 preferences and needs, respecting their values and wishes.<\/li>\n<li>Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards.<\/li>\n<li>Participate in on-call rotation as required to provide care and support after hours.<\/li>\n<li>Perform all other duties assigned.<\/li>\n<\/ul>\n<p>\u00a0<\/p>\n<p><i><strong>Job Qualifications:<\/strong><\/i><\/p>\n<ul>\n<li>Valid license as a Registered Nurse (RN) in the state where work is performed.<\/li>\n<li>2 or more years of nursing experience, preferably with a focus on hospice, palliative care, or related fields.<\/li>\n<li>Strong clinical skills and knowledge of nursing principles, procedures, and best practices in long-term care.<\/li>\n<li>Excellent communication and interpersonal skills, with the ability to interact effectively with residents, families, caregivers, and healthcare professionals.<\/li>\n<li>Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment.<\/li>\n<li>Understanding of regulatory requirements in hospice settings and nursing practices.<\/li>\n<li>Ability to build rapport with patients through a compassionate and caring demeanor.<\/li>\n<\/ul>\n<p>\u00a0<\/p>\n<p><i><strong>Physical Requirements:<\/strong><\/i><\/p>\n<ul>\n<li>Ability to lift and transfer patients, with or without assistance, up to 50lbs.<\/li>\n<li>Capability to perform physical tasks such as bending, standing, walking, and climbing stairs for extended periods.<\/li>\n<li>Strength to handle medical equipment and assist patients with mobility and physical activities.<\/li>\n<li>Visual acuity and dexterity for performing tasks such as reading medical records, administering injections, and handling delicate medical instruments.<\/li>\n<li>Ability to respond swiftly and efficiently to emergency situations.<\/li>\n<li>Capacity to work in various home environments which may include navigating stairs, tight spaces, or uneven surfaces.<\/li>\n<li>Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor\/manager.<\/li>\n<li>The job requires accepting criticism and dealing calmly and effectively with high-stress situations.<\/li>\n<li>The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without &#8220;giving out&#8221; or fatiguing.<\/li>\n<li>The ability to keep your hand and arm steady while moving your arm or while holding your arm and hand in one position.<\/li>\n<\/ul>\n<p>\u00a0<\/p>\n<p><strong>Schedule:<\/strong><\/p>\n<ul>\n<li>Monday-Friday 8am &#8211;
