Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.<br \/><strong>Who You Are:<\/strong> You are a motivated Occupational Therapist who shares our vision of delivering in-home care. You treat patients in a caring and compassionate manner and help them achieve their goals of staying in their own home whilst receiving treatment.<br \/><strong>Travel<\/strong>: This role comprises 100% travel within our service region. You will be traveling to patients\u2019 homes.<br \/><strong>Service Region:<\/strong> We have several counties that we service across Northern California specifically Butte County, Shasta County and Yuba Sutter counties and surrounding communities.\u00a0<br \/><strong>Schedule<\/strong>: We are hiring for Full-Time 40 hours per week M-F 8am-5pm or Part-Time\/Per-Diem role of between 10-20 hours per week, with the possibility of expanded hours in the future.\u00a0<br \/><strong>Duties and Responsibilities:<\/strong> To perform this job successfully, you must be able to perform the following duties successfully:<\/p>\n<ul>\n<li>Evaluate patients upon physician diagnosis<\/li>\n<li>Assess patient and identify deficits impacting self-care, functional mobility, behavior, cognition and patient\u2019s ability to function in their life\u2019s role<\/li>\n<li>Deliver occupational therapy to patients in their own homes<\/li>\n<li>Help patients improve their motor skills, coordination, and balance through therapeutic exercises and activities<\/li>\n<li>Assist patients in developing or regaining daily living skills, such as dressing, bathing, and eating<\/li>\n<li>Teach patients how to use adaptive equipment and assistive devices to enhance their independence<\/li>\n<li>Collaborate with other healthcare professionals, such as physicians, nurses, social workers, physical therapists and speech therapists, to coordinate patient care<\/li>\n<li>Monitor and document patients&#8217; progress during therapy sessions<\/li>\n<\/ul>\n<p><strong>Essential Requirements:<\/strong><\/p>\n<ul>\n<li>One year experience as a home health care provider<\/li>\n<li>Current California State license as an Occupational Therapist and stays updated through independent study and continuing education<\/li>\n<li>Current CPR certification<\/li>\n<li>Valid California Driver\u2019s License<\/li>\n<li>Eligible to be insured under our liability policy with a clean DMV report<\/li>\n<li>Your own vehicle and proof of current auto insurance<\/li>\n<li>COVID vaccinated or have an exemption. PPE is provided to all employees<\/li>\n<\/ul>\n<p><strong>Required Skills and Abilities:<\/strong><\/p>\n<ul>\n<li>Demonstrated technical competence and accountable for work output.<\/li>\n<li>Demonstrates proficiency in performing occupational therapy procedures and modalities and in assessing the effects of these<\/li>\n<li>Demonstrates sensitivity with patient, family, and their medical team<\/li>\n<li>Ability to work with others, at all levels within the organization, and collaborate effectively.<\/li>\n<li>Above-average interpersonal, problem-solving, and written and oral communication skills.<\/li>\n<li>Professional behavior when on duty<\/li>\n<li>Ability to maintain patient records and able to organize daily schedule<\/li>\n<\/ul>\n<p><strong>Physical Requirements:<\/strong> Conditional upon job offer is successfully passing a medical exam. Physical requirements for this role specifically include<\/p>\n<ul>\n<li>Sits, stands, bends, lifts, walks, and moves intermittently during working hours.<\/li>\n<li>Able to lift to 50 pounds without assistance.<\/li>\n<li>Able to drive intermittently on a daily basis<\/li>\n<\/ul>\n<p><strong>Compensation:<\/strong><br \/>Pay range is $55.00 to $65.00 per hour.
