We strive to enhance the health and well-being of the residents of Portland in collaboration with our community, state, and federal partners.<\/p>\n<p>\u00a0<\/p>\n<p><strong>Job Summary:<\/strong><\/p>\n<p>\u00a0<\/p>\n<p>The Barron Center is a long-term care facility, owned and operated by the City of Portland, and is Maine\u2019s largest skilled nursing facility. This position involves professional social work with long-term care residents and their families, assisting in their adjustment to the facility, and working as a team member to provide for and assure quality of life for facility residents. Qualified candidates will have at least one year of experience in long-term care or within geriatric care work, including knowledge of community resources. The Licensed Social Worker designation is required for consideration and hire.<\/p>\n<p>\u00a0<\/p>\n<p><strong>Duties &amp; Responsibilities:<\/strong><\/p>\n<p>\u00a0<\/p>\n<p>\u2022 Shares responsibility for telephone and walk-in referrals.<\/p>\n<p>\u2022 Provides appropriate community referrals.<\/p>\n<p>\u2022 Meets with residents and their family upon admission, determines psychosocial needs, and formulates an appropriate care plan.<\/p>\n<p>\u2022 Visits residents on a regular and as-needed basis.<\/p>\n<p>\u2022 Maintains contact with families, involving them in resident\u2019s care as appropriate.<\/p>\n<p>\u2022 Acts as an advocate for residents and their rights.<\/p>\n<p>\u2022 Participates in team conferences and psychiatric consultations.<\/p>\n<p>\u2022 Works as discharge planner arranging for services at time of discharge.<\/p>\n<p>\u2022 Responsible for preparing and maintaining social service documentation in accordance with State regulations.<\/p>\n<p>\u00a0<\/p>\n<p><strong>Required Skills &amp; Experience:<\/strong><\/p>\n<p>\u00a0<\/p>\n<p>\u2022 Bachelors Degree in Social Work or a related field. Licensed Social Worker designation is required.<\/p>\n<p>\u2022 Minimum one year experience in long term care or geriatric care work<\/p>\n<p>\u2022 Experience in family or group counseling<\/p>\n<p>\u2022 Working knowledge of social services system and community social resources<\/p>\n<p>\u2022 Ability to prepare complete and valid case records<\/p>\n<p>\u2022 Ability to apply policies and procedures to specific cases.<\/p>\n<p>\u2022 Ability to assist families and individuals with social and economic planning.<\/p>\n<p>\u2022 Ability to establish and maintain effective working relationships with other social workers, institutional and public officials, and other community contacts<\/p>\n<p>\u00a0<\/p>\n<p><strong>Expected Hours\/Schedule:<\/strong><\/p>\n<p>\u00a0<\/p>\n<p>Monday-Friday, 8am-4pm.<\/p>\n<p>\u00a0<\/p>\n<p><strong>Application deadline extended &#8211; applications accepted until June 12, 2026.<\/strong><\/p>\n<p>\u00a0<\/p>\n<p>The successful candidate will be subject to a pre-employment physical exam.
