Our team helps individuals obtain housing, employment, mental health services, and more.<\/p>\n<p><u>What to expect.<\/u><\/p>\n<p>This is a direct care opportunity to provide support for mental health treatment. You will <strong>assess and monitor<\/strong> client needs, develop <strong>service plans<\/strong>, connect clients to <strong>community services and resources<\/strong>, and <strong>advocate for your clients<\/strong> as necessary.<\/p>\n<p>Additional responsibilities will include:<\/p>\n<ul>\n<li>Assisting clients through <strong>program enrollment and orientation<\/strong>.<\/li>\n<li>Coordinating client medical appointments, transportation, housing, money management, etc.<\/li>\n<li>Developing and assisting with client <strong>medication management plan<\/strong>.<\/li>\n<li>Obtaining and <strong>maintaining entitlements<\/strong> for clients.<\/li>\n<li>Maintaining accurate and complete documentation.<\/li>\n<li>Developing and <strong>coordinating transitions plans and program discharge<\/strong>\n<\/li>\n<\/ul>\n<p><u>What you\u2019ll get from us.<\/u><\/p>\n<p>At Sheppard Pratt, you will work alongside a multi-disciplined team led by a bold vision to change lives.
