Provides quality Physical Therapy services in a timely manner to patients according to a written plan of care.<\/p>\n<p>\u00a0<\/p>\n<p><strong>Professional Duties<\/strong><\/p>\n<ul>\n<li>Maintain member confidentiality at all times (HIPAA).<\/li>\n<li>Provides direct planned patient therapy services (within scope of practice) under the direction and orders of a Physician\/Practitioner and according to agency policies and patient plan of care.<\/li>\n<li>Completes initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate times.<\/li>\n<li>Develops and revises the plan of care, in consultation with the physician\/practitioner and other team members as appropriate, which establishes goals and incorporates therapeutic, preventative and rehabilitative therapy services as appropriate.<\/li>\n<li>Provides an evaluation of DME assignments\/needs as appropriate.<\/li>\n<li>Advises and consults with patient, family, and other team members as appropriate regarding safety, plan and outcome of care, and discharge planning.<\/li>\n<li>Provides health teaching to patient and\/or caregiver regarding treatment and plan of care. Education will utilize terms and teaching methods that are customized to the learning needs of the patient and\/or caregiver.<\/li>\n<li>Provides accurate and timely documentation consistent with the plan of care, this may include preparing clinical and progress notes as appropriate. Documentation will be completed in a timely manner and according to the organization\u2019s documentation expectations.<\/li>\n<li>Performs supervisory visits of Therapy Assistants and Home Health Aides as appropriate.<\/li>\n<li>Participates in team meetings and in-service programs as required.<\/li>\n<li>Weekend shift coverage as needed.<\/li>\n<li>Performs other duties as assigned.<\/li>\n<li>Serve as an ambassador for Desert Oasis Healthcare and Family Hospice Care at all times and positively shape the customer experience<\/li>\n<li>Be knowledgeable about your job so that you can serve others effectively<\/li>\n<li>Let your patients, customers and co-workers know what they can expect from you and follow through<\/li>\n<\/ul>\n<p><strong>Qualifications<\/strong><\/p>\n<ul>\n<li>High School Diploma or equivalent. Bachelor&#8217;s (BSPT), Master&#8217;s (MPT), or Doctor of Physical Therapy (DPT) degree in Physical Therapy required.<\/li>\n<li>Current California Physical Therapy License.<\/li>\n<li>Minimum of 1 to 2 years recent acute care experience required. 1-2 years in Home Health preferred.<\/li>\n<li>Basic computer skills.<\/li>\n<li>Ability to manage time and stay organized.<\/li>\n<li>Good communication skills, detail oriented, critical thinking and problem solving skills.
