— Enter your First Name and Last Name in the respective fields below.
— Enter only the seven digits of your Student ID (not the s) in the field below.
— Select the Month, Day, and Year of your Date of Birth from the available menus.
— Enter the security word as displayed in the available field.
— Click Submit Your Information.
